The more information you provide on the clinical questionnaire, the better we can pair the right materials to your body and your lifestyle needs. The questionnaire will be included in the foot impression kit sent in the post after you place your order. Once the lab receives your foot impression kit, we will scan it with a 3D laser and start your custom orthotic design.
It is VITAL that you send a trace of the inside of the shoes/boots that the orthotics will be used in. Most sports shoes have a narrow arch and do not tolerate orthotics well. If there is a removable insole, please draw around it. If not, trace around the sole of the shoe but note on the tracing whether it is an insole or the outer sole so that we can apply the relevant calculations.
